Residence Registration Form [Meldezettel]

The Residence Registration Form [Meldezettel] is the form which the Residence Registration Authorities [Gemeinde] use to enter the registration data into the Central Register of Residence Registrations [Zentrales Melderegister].

The following information must be included on the form:

• Name (including all former names)
• Date of birth
• Place of birth
• Sex
• Nationality

However, you are not obliged to give information about your religion.

Please note: The registration form must be signed

• in the case of a primary apartment leasehold by the real estate management company,
• in the case of a sublet, by the primary leaseholder, and
• in the case of a condominium or house, by the owner.

The registration form can be obtained

• directly at the registration office,
• at some tobacco shops, or
• as the online form "[Meldezettel]" (download application form).
